New Student ID Card

This form is to be used by all new and returning students studying at Avondale University (Avondale), despite the course, campus or attendance mode.

Instructions

Before requesting your Student ID Card, please ensure that you have:

1. Accepted your offer and enrolled in at least one (1) unit
2. Prepared your identification documents to evidence your identity, to enable Avondale to fulfil its obligations under applicable privacy laws.
3. A recent colour photo of yourself, which must meet the requirements listed here

Personal information collected in this form will be managed in accordance with Avondale’s Privacy Policy (Higher Ed).

NOTE: Please be aware that if you provide falsified documents in support of your application, this will result in one of a range of penalties, from failure in the course to suspension or exclusion. In some cases, the matter could also be reported to ICAC.
